About Tesamorelin.
Tesamorelin is a synthetic GHRH analogue holding Marketing Authorisation in the United States for the reduction of visceral adipose tissue in HIV-associated lipodystrophy. It is also of interest off-label for cognitive support in older adults, supported by NIA-funded research.
Tesamorelin is more stable than native GHRH and produces a robust GH pulse. Its strongest evidence base is for targeted visceral adipose reduction; the cognitive evidence is promising but earlier-stage.
Tesamorelin binds the GHRH receptor on pituitary somatotrophs, triggering GH release. Increased GH and downstream IGF-1 stimulate lipolysis preferentially in visceral fat depots, mediated by the activation of hormone-sensitive lipase.
Subcutaneous injection, typically 1–2 mg once daily, preferably at bedtime, for protocols of 12–24 weeks with re-evaluation.
